I have also ran a simulation without the boundary condition on the fan plane and still had divergence error.
The simulation worked for low velocity but when it comes to high velocity around Mach 0.8 i start to get these errors.

I’m not exactly sure what software was used in this latest image that was posted, however I would highly recommend that you search for information on what exacly a “Pressure outflow” boundary condition means and in what cases it’s used.
Nomenclature issues are common, and, although similar, SimScale’s pressure outlet does not behave mathematically the same as a pressure outflow BC.

From this paper they used pressure outlet boundary condition using Star CCM++.

Feel free to continue using this approach, however, for OpenFOAM purposes, I can tell you for sure that it’s very unstable and not recommended at all.
I hope this clarifies any misunderstandings related to the nomenclature of BCs.
